DCMP Nurse Case Managers also provide oversight of pharmaceutical care to ensure cost-effective and therapeutically appropriate medication management.

• The main objective that Pharmaceutical Case Management provides is the collaboration between the prescribing Physician and the Nurse Case Manager to explore alternative, economical medication options while prioritizing therapeutic effectiveness and patient safety.  
• The Nurse Case Manager schedules face-to-face meetings with the providers to review and adjust medication regimens, evaluate current prescriptions and to seek opportunities to use generics, formulary alternatives or modification in dosage when applicable.  
• Written documentation of any changes made to the treatment plan is provided by the Nurse Case Manager to all parties involved.
