These cases typically involve extensive physical or cognitive impairments, multiple medical providers, and prolonged recovery timelines.
Responsibilities of Catastrophic Nurse Case Managers:
Compassionate Support: Provide empathetic advocacy for patients facing significant physical and emotional challenges.
Integrated Coordination: Facilitate communication between medical professionals, insurers, and family members.
Holistic Approach: Address both the medical and psychosocial components of recovery.
On-Site Involvement: Conduct face-to-face meetings during critical phases of treatment and rehabilitation.
Long-Term Planning: Assist with care planning, home modifications, rehabilitation, and ongoing support services.
These services are particularly suited for cases involving traumatic brain and/or spinal cord injuries, amputations, severe burns, and other debilitating conditions.
