Field Case Management (FCM)

DCMP’s Field Nurse Case Management (FCM) program supports injured workers through the complexities of recovery by offering coordinated care and compassionate, hands-on guidance. All DCMP FCMs conduct in-person meetings with clients and their healthcare providers to offer immediate support, streamline communication, and tailor recovery plans to meet each individual’s medical condition and personal needs.

DCMP nurse case managers work collaboratively with claims adjusters, employers, medical providers, and injured employees to facilitate every aspect of the process. This collaborative approach helps expedite return-to-work timelines and improve clinical and functional outcomes. The primary goal is to ensure that injured workers receive appropriate care while focusing on recovery and timely reintegration into the workforce.

Key Responsibilities of Field Case Managers:

  • In-Person Engagement: Meet with injured workers at medical appointments, their homes, or workplaces to evaluate progress and address concerns.
  • Medical Oversight: Review treatment plans and collaborate with providers to ensure alignment with recovery goals.
  • Communication: Serve as the liaison between all parties involved ensuring consistent and transparent communication.
  • Education and Guidance: Educate injured workers about their conditions, treatment expectations, and workplace accommodations.
  • Advocacy: Promote medically necessary, cost-effective care that supports recovery and return to work.
  • Documentation: Maintain comprehensive records of assessments, interventions, and communications.
  • Return-to-Work Planning: Partner with employers to support job modifications or if required, initiate vocational rehabilitation.
  • Barrier Identification: Recognize and help resolve social, psychological, or logistical barriers that may delay recovery.
  • Referral Coordination: Arrange access to specialists, therapies, and other medical services as needed.
  • Case Conferences: Participate in interdisciplinary meetings to align recovery strategies.
